Applicable scenarios
The selection of alginate dressings should be based on wound type, exudate volume and morphological characteristics.
The following are key indications:
1. Moderate to severe exudative wounds
2. Suitable for wounds with a lot of exudate such as pressure sores (stage 3/4), lower limb venous ulcers, surgical dehiscence wounds, burns and donor sites. Its absorption capacity can reach 15-30 times its own weight, effectively draining exudate and maintaining a moist environment.
3. Irregular cavity wounds
Such as deep cavities, abscesses, sinuses or undermining wounds, strip dressings can fill and fit, reduce gap residues, and prevent exudate accumulation.
4. Infected wounds (need to work with antibacterial treatment)
Infected wounds containing slough or necrotic tissue can be fixed with gel and inhibited from reproduction. The silver version can enhance the antibacterial effect.
5. Chronic wound management
For long-term wounds such as diabetic foot ulcers and chronic venous ulcers, granulation growth and epidermal regeneration are promoted through a moist healing environment. 6. Hemostasis needs
For wounds with slight bleeding or oozing, calcium ion release can activate prothrombin to assist hemostasis.
7. Postoperative wound packing
For example, postoperative wounds around the anus can reduce exposure of nerve endings and pain, and there is no adhesion when changing dressings.
Contraindications and limitations
The following situations should be avoided:
1. Dry or low-exudation wounds: Wound dehydration may occur due to excessive absorption.
2. Deep wounds (such as penetrating wounds, arterial/venous ulcers): Vascular lesions or deep tissue injuries should be treated first.
3. Uncontrolled severe infection: Systemic anti-infection treatment is required first.
4. Wounds covered with large areas of burns or necrotic tissue: Tissue edema or infection risk may be aggravated.
Selection should be combined with dynamic assessment of the wound:
Exudation volume: It is preferred to be used in the moderate to severe exudation stage, and other moisturizing dressings need to be replaced after the exudation decreases.
Morphological adaptability: Strips are preferred for cavities or complex wounds, and sheets are used for superficial wounds.
Infection status: Infected wounds require combined antibacterial treatment and close monitoring.

| Antimicrobial | Form a physical barrier to reduce the risk of infection | YES |
| benefits | Promote autolytic debridement The moist environment accelerates the softening and shedding of necrotic tissue, reducing the need for mechanical debridement. |
Functional mechanism Gel barrier isolates contamination The formed gel layer physically isolates the inner wall of the cavity from external pollutants (such as feces and bacteria), reducing the risk of secondary infection. Continuous release of calcium ions to stop bleeding The released calcium ions activate platelets and coagulation factors, and control the micro-bleeding of the wound surface (especially suitable for postoperative hemorrhagic cavities). Promote moist healing environment Maintain the moist state of the wound cavity, accelerate the growth of granulation tissue and epithelial crawling |
| Advantages | Directly cover the wound surface, absorb exudate to form gel, and maintain a moist healing environment. | It can be cut to tightly fill the cavity, absorb deep exudate, reduce dead space, and promote granulation growth. |
| Contraindications | Infected wounds: Ordinary alginate dressings can be used for infected wounds, but antibacterial treatment (such as combined use of antibacterial agents) is required at the same time; if the risk of infection is high, it is recommended to use alginate dressings containing silver ions. Replacement frequency: Adjust according to the amount of exudate (frequent replacement is required when there is a lot of exudate to avoid saturation). Cutting requirements: Sheet dressings need to extend 1–2 cm beyond the edge of the wound, and strip dressings should not be filled too tightly. |
